The first step to becoming a Medicare provider is to complete your enrollment application and receive your Provider Transaction Access Number (PTAN) and National Provider Identifier (NPI). Watch a short video for an overview of the provider enrollment process and then please complete a brief survey.

How Do I Become a Medicare Provider?

Fastest Method

  1. Obtain a National Provider Identifier (NPI). NPIs are a unique health care provider identifier and are issued through the National Plan and Provider Enumeration System (NPPES).

  2. Complete the Medicare Enrollment Application. You can enroll in Medicare using PECOS, the online Medicare enrollment system.

  1. Download and complete the correct form based on what type of provider you are:

    • Organizations — CMS-855B (PDF, 833 KB)
    • Physicians and nonphysician practitioners — CMS-855I (PDF, 378 KB)
    • Physicians and nonphysician practitioners reassigning Medicare benefits — CMS-855R (PDF, 176 KB)
